Financial Considerations

We recognize that financial considerations are very important to our patients. In an effort to provide the highest level of comprehensive care and service to our patients, our policy since 1947 has been to treat all patients regardless of their ability to pay. If you will be coming to see our physicians for the first time, or if you have had a change in insurance, please remember to bring along your insurance card so that we may photocopy it. If your plan requires a written referral or authorization from your primary care physician to see a specialist, please obtain it and bring it with you to your visit. Regretfully, if you do not have your referral, we will have to reschedule your visit.

If your visit is related to an automobile accident, please bring the name and address of the automobile insurance carrier, name of the insured, and claim number.

If your visit is related to a legal claim, please provide for us the name and address of your attorney. Be advised that we do not bill attorneys for medical services. However, your signature of an authorization and assignment form (A & A) will enable us to wait for payment of your services until a legal settlement is made.

We will be glad to submit an insurance claim to your health plan for services rendered. Co-pays and deductibles are due at the time of your visit. We accept cash, checks, Visa, Mastercard, and Discover for payment. If you require special payment arrangements, please speak to our accounts receivable manager prior to your visit.
