Become a New Patient

We are currently accepting new patients into our practice. Thank you for considering us. To become a new patient:

If you weren’t able to download and completed the forms on the Patient Portal prior to your first visit, including the Authorization and Consent for Treatment form, we will ask you to complete those forms when you arrive at our office for your appointment.

Please arrive 15 minutes prior to your scheduled appointment to ensure your registration is complete before meeting with your new provider and there are no delays in care.

Please bring the following documentation with you:

  • Your insurance card & valid photo ID

  • Any MRIs, CTs, and X-rays of the brain and/or spine

  • Office co-pay and your referral from your primary care for all straight HMO plans

In an effort to respect the time of our patients, our staff strives to maintain our schedule and reduce patient waiting times.

We will make every effort to see patients, who are delayed and arrive late for an appointment, the same day. Please be advised that additional wait times may apply or the appointment may need to be rescheduled.
